深低温+100%甘油+RPMI 1640合成细胞培养液保存羊膜联合角膜缘干细胞移植在眼表重建中的应用

摘  要:

目的:评价深低温+100%甘油+RPMI 1640合成细胞培养液保存羊膜联合角膜缘干细胞移植在重建眼表结构中的治疗效果: 方法:①实验对象:选取解放军南京军区福州总医院眼科中心2000-2006年收治的严重眼表烧伤患者29例34眼。实验经医学伦理委员会批准,剖腹产孕妇的胎盘由本院妇产科提供,孕妇均签署知情同意书。离体6h以内的同种异体新鲜角膜由福建省红十字会提供。②实验方法:钝性分离羊膜与绒毛膜,将羊膜上皮面朝上平铺于去除胶面的眼科手术薄膜纸片上,剪成3cm×4cm大小,置于100%甘油+RPMI 1640合成细胞培养液各5mL的无菌小瓶中,-80℃冷藏备用。在供体眼球角膜缘前1mm处透明角膜上、角膜缘后2mm处巩膜上作适当深度的环形划界,然后剖出带有浅层基质的角膜缘上皮移植片备用。严重眼表烧伤组织,将备好的羊膜植片上皮面朝上平铺于眼表面,依结膜缺损区大小修剪,缝合固定于近角膜缘的浅层巩膜上,使羊膜紧贴于角膜表面,然后将羊膜边缘与球结膜对位缝合。再将备好的角膜缘七皮移植片与植床角膜缘对位固定缝合。此外,4眼伴轻度眼睑闭合不全者同时行眼睑缘粘连性缝合。 结果:严重眼表烧伤患者29例34眼中,除眼睑缘粘连性缝合的4眼外,余30眼(88.24%)术后7~14d角膜水肿消退,14~21d上皮修复角膜全表面,结膜表面光滑,无感染发生;术后3~6个月角膜新生血管明显减少,角膜透明度改善,视力提高3~5行;经过22.8个月随访,眼表面稳定。睑缘粘连性缝合的4眼(11.76%)发生羊膜植片急性排斥反应,羊膜融解,改行深板层角膜移植,2个月后观察眼表亦光滑,无睑球粘连发生。 结论:深低温+100%甘油+RPMI 1640合成细胞培养液保存羊膜联合角膜缘干细胞移植手术,既发挥了羊膜的生物学特性,又提供了角膜缘活性细胞,能快速、稳定促进角膜更新机制的恢复,加速角膜表面重新上皮化,是治疗严重眼表烧伤有效、可行的手术方式。[著者文摘]

Abstract:

AIM: To evaluate the effect of cryopreserved amniotic membrane in 100% glycerine and RPMI 1640 medium combined with limbal stem cells allograft transplantation in ocular surface reconstruction. METHODS: ①Totally 34 eyes of 29 patients with severe ocular surface burn were recruited by the Ophthalmic Center in Fuzhou General Hospital of Nanjing Military Area Command of Chinese PLA from 2000 to 2006. The experiment was approved by the hospital ethics committee. With the informed consents of pregnant women following cesarean delivery, the placenta was offered by the Department of Gynecology and Obstetrics in the same hospital. Fresh cornea within 6 hours ex vivo was provided by the Red Cross Society of China Fujian Branch.②Amniotic membrane was separated from chorionic vesicle by a blunt dissection. Facing upward, the epithelium of amniotic membrane was put on the thin-film for ophthalmologic operation by hautchen technique, and then placed in the aseptic bottle containing 5 mL 100% glycerine and 5 mL RPMI 1640 medium for preservation at -80 ℃, in a size of 3 cm × 4 cm. Toroid demarcation was cleared on transparent cornea 1 mm ahead and tunica dura 2 mm behind limbus of cornea to expose the epithelial with superficial matrix. For severe burnt patients, vascularized scars and damaged matrix tissues were eliminated, then amniotic membrane was upwards hatched on ocular surface and fixed on the superficial tunica dura in accordance with the defect area. Amniotic membrane was force to stick cornea tightly, with the apposition suture between amniotic membrane and tunica conjunctive bulbi. Afterwards the cryopreserved amniotic membrane was combined with limbal stem cells allograft for transplantation. Additionally four eyes with hypophasis underwent anchyloblepharon suturation at the same time. RESULTS: Thirty eyes (88.24%) of 29 patients with severe ocular surface burn were averagely followed up for 22.8 months. At 7-14 days after operation, the corneal edema was extinct. At 14-21 days, the corneas reepithelialized without infection. At 3-6 months, the corneal new vessels distinctively reduced, their clarity was improved, and the visual acuity was elevated 3-5 lines. As for 4 eyes (11.76%) occurred acute rejection reaction, deep layer of corneal transplant was conducted following anchyloblepharon suturation, and recovered two months later. All the 34 eyes resulted in the restoration of a normal and stable ocular surface. CONCLUSION: Cryopreserved amniotic membrane in 100% glycerine and RPMI 1640 medium combined with limbal stem cell allograft transplantation is a useful technique in promoting the rapid and stable reepithelialization of ocular surface following severe ocular burn. It not only utilizes the bionomics of the amniotic membrane, but also provides the limbal stem cells. Thus it is a feasible and effective way for therapy of severe ocular surface.[著者文摘]
