摘 要:
目的 通过某院普通外科围手术期抗菌药物预防使用的干预对照研究,探索该院围手术期抗菌药物合理使用方案实施的可行性。方法 选取普通外科2003年10月出院的Ⅰ、Ⅱ类切口手术病例208例为非干预组,2004年10月出院的Ⅰ、Ⅱ类切口手术病例184例为干预组,比较两组药费、抗菌药物使用合理性、平均住院天数及手术感染率。结果 干预组平均药费为2834.5元,平均抗菌药费为782.8元,均显著低于非干预组平均药费4086.6元和平均抗菌药费1793.4元(分别P〈0.05,P〈0.01);抗菌药物预防使用平均天数,干预组为(3.1±1.4)d,非干预组为(5.9±2.6)d,两组差异有高度显著性(P〈0.01)。采取术前30min预防用药比率由干预前8.17%上升至干预后99.46%,差异有高度显著性(P〈0.001)。干预组与非干预组平均住院天数分别为14.1d、13.6d,手术部位感染率分别为0与0.48%,两两比较,均无差异(P〉0.05)。结论 该院实施的合理用药方案降低了患者的药费,特别是抗菌药费,提高了术前预防用药的合理性,对住院时间和手术切口感染率无影响,具有有效性和可行性。[著者文摘]

Control study on the interfering with rational application of perioperative antimicrobial agents in general surgery

Abstract:
Objective To evaluate the feasibility of rational application of perioperative antimicrobial agents in a hospital by control study on the interfering with rational application of perioperative antimicrobial agents in a general surgery. Methods Two hundred and eight patients with grade Ⅰ or Ⅱ incision operation who were discharged from hospital on October 2003 were as non-interfering group, 184 patients with grade Ⅰ or Ⅱ incision operation who were discharged from hospital on October 2004 were as interfering group, the cost and rationality of antimicrobial agents, the average hospitalization days and surgical wound infection rates in two groups were compared. Results The average cost of all medicine and antimicrobial agents were 2 834. 5 and 782. 8 yuan respectively in interfering group, which were obviously lower than 4 086. 6 and 1 793. 4 yuan respectively in non-interfering group(P〈0. 05; P〈0. 01 respectively) ; the average antimicrobial application days in interfering group and non-interfering group were (3. 1 ± 1.4) days and (5. 9± 2. 6) days respectively, there was significant difference between two groups(P〈0. 01). Prophylactic antimicrobial application rate 30 minutes before operation rose from 8. 17 % in non-interfering group to 99. 46% in interfering group, there was significant difference between two groups(P〈0. 001). The average hospitalization days in interfering group and non-interfering group were 14. 1 and 13. 6 days respectively, surgical wound infection rate was 0 and 0. 48% respectively, there was no obvious difference between two groups(P〉0. 05). Conclusion The rational antirnicrobial application in the hospital can reduce the cost of drugs, especially antimicrobial agents, but has no influence on hospitalization days and surgical wound infection rate, which is effective and practical.[著者文摘]
