摘 要:
目的研究脑梗死急性期血压的变化规律，探讨脑梗死急性期血压调控对预后的影响，为临床治疗提供依据。方法以北华大学附属医院神经内科发病24h内入院的脑梗死患者为研究对象，记录其人口特征、既往病史、入院时血压、CSS评分、Barthel指数，将患者随机分为两组：实验组和对照组。对实验组患者的血压进行调控，并连续观察两组患者入院后1周内的血压情况，观察两组患者1周内的血压变化规律，及15，30，90，180d两组患者的死亡、复发、残疾（CSS评分和Barthel指数）情况，比较两组患者预后的差异。结果两组患者急性期血压变化的规律不同，两组患者的近期和远期预后均有显著性差异。结论脑梗死患者发病时血压增高，实验组血压24h内迅速下降，第4天稳定；对照组血压3d内迅速下降，第5天稳定；实验组预后优于对照组，脑梗死急性期宜适量调控血压：病灶数量影响患者预后，两者呈正相关。[著者文摘]

Abstract:
Objective To investigate the change regularity in blood pressure of patients with acute ischemic stroke and independent factors related to BP levels; To prognosis. Methods Consecutive register patients with Neurology evaluate the relationship between BP in acute stage and acute ischemic stroke in the 24 hours of Department of , Affiliated Hospital of Beihua University. Taking their characteristics, blood pressure, CSS grade, Barthel index number, and dividing them in to two groups, experiment group and control group. Regulating BP in the experiment group. Observing their change of BP in the first week and analyzing its regular. Death, recurrence and disability were followed up at day 15, 30, 90, 180. And comparing the differences of the two groups. Results The regularity of BP change is different between two groups. The prognosis of near future and long-term between two groups are different. Conclusion BP of patients with acute ischemic stroke is higher than normal . BP is falling in 24 hours with experiment group, and it is stable in the fourth day. BP is falling in 3 days with control group, and it is stable in the fifth day; The prognosis of experiment group is better than the control group. BP in acute ischemic stroke should be prognosis. regulated; The number of infection is also influencing the prognosis.[著者文摘]
